As the number of Blacks, Latinos, Asians and American Indians covered by the Affordable Care Act increases, learn how to best understand patient values and beliefs, attract and retain representative staffs, and mobilize community outreach for sustainable wellness programs. There will be specific emphasis on diseases impacting these communities, such as diabetes and high blood pressure.

Dr. Jennifer Mieres
As leader of the health system’s Office of Community and Public Health, Dr. Mieres has oversight of the Katz Institute for Women’s Health, all of North Shore-LIJ’s health and wellness, community education and healthcare access programs, as well as corporate social responsibility. She is also the health system’s chief diversity and inclusion officer, and medical director of the Center for Learning and Innovation, North Shore-LIJ’s corporate university. Along with other staff, Dr. Mieres has been collaborating with facilities across the health system and communities to enhance diversity, inclusion and health literacy efforts to ensure North Shore-LIJ provides culturally competent, health-literate care.
Dr. Mieres was a member of the faculty of the New York University Langone Medical Center, and served as director of nuclear cardiology from 2006-2010, a position she previously held at North Shore University Hospital. In 2009, she served as the first female president of the American Society of Nuclear Cardiology. Dr. Mieres received her medical degree from Boston University and a bachelor’s degree from Bennington College, Bennington, Vermont. She completed her residency as well as fellowships in cardiology and nuclear cardiology at St. Luke’s/Roosevelt University Hospital Center, New York.
Dr. Richard Amiraian
As Corporate Vice President, Primary Care, Continuum Health Partners, Dr. Amiraian is responsible for primary care transformation and developing the medical enterprise including leading the implementation of Clinical Integration initiatives, the Medical Home Initiative and the Continuum Provider Partners, IPA; for which he serves as Chairman of the Board of Managers and as Co-chair of the Clinical Integration Committee.
Dr. Amaraian has a bachelor’s degree from Union College and completed his M.D. at SUNY at Buffalo School of Medicine. He did post-doctoral training at the University of Rochester.
Dr. Naa-Abia Sophia Casely-Hayford
Dr. Naa-Abia Sophia Casely-Hayford is the Associate Medical Director for Ambulatory Care Services and the Medical Director of Employee Health Services at St. Luke’s-Roosevelt Hospital Center (SLRHC). In addition, she practices medicine at St. Luke’s Medical Group, which is located in Central Harlem.
Dr. Casely-Hayford received her medical degree and her Master’s in Public Health from Columbia University College of Physicians and Surgeons in 1994 and the Mailman School of Public Health in 1995, respectively. She went on to complete her residency in Internal Medicine at New York Presbyterian Hospital in 1998. Dr. Casely-Hayford joined the faculty of the Division of General Internal Medicine at SLRHC in 2002.
At SLRHC, Dr. Casely-Hayford was the Lead physician for the Refugee Medical Screening Program and the Academic Chronic Care Collaborative, which ultimately demonstrated the greatest improvement in diabetes outcome measures of all 26 hospitals that participated in this collaborative with the American Association of Medical Colleges. She was also the physician champion for the Medical Home Initiative at St. Luke’s Medical Group. Through this endeavor, St. Luke’s Medical Group achieved NCQA Patient Centered Medical Home Level 3 in 2011.
